Key criterion 1: Quality and innovation
HDFT consistently delivers strong performance across the 0-25 Family Health Service. Robust governance and data-quality arrangements are in place. Quality and safety remain strong, with routine monitoring of incidents and complaints, low levels of formal escalation and consistently positive service-user feedback. HDFT continues to invest in quality improvement, supporting service development, evaluation and continuous improvement.
Key criterion 2: Value
The service supports over 150,000 children and young people, providing universal and targeted support in response to significant levels of need and health inequality across County Durham. HDFT demonstrates financial responsibility through transparent monthly reporting on staffing and workforce pressures and has reduced its management fee over the contract term, supporting continued cost efficiency.
Key criterion 3: Integration, collaboration and service sustainability
HDFT works effectively with Public Health, Early Help, Children's Social Care, Maternity Services and Family Hubs, supporting integrated pathways and a consistent "no wrong door" approach for families.
HDFT responds collaboratively to system change and emerging need through joint pathways, strengthened referral arrangements and active participation in strategic public health partnerships.
Key criterion 4: Access, inequalities and choice
HDFT supports access and reduces inequalities through a tiered model of universal, enhanced and targeted provision, enabling early identification of need and preventative support.
Services are delivered flexibly across homes, community venues, Family Hubs and schools, supported by digital provision including a 24/7 health app, online resources and practitioner-led health chat.
