Abstract

The NHS Wales Shared Services Partnership, hosted by Velindre NHS Trust are acting on behalf of Cardiff and Vale University Local Health Board who are seeking to establish a contract for Social Prescribing Model in South and East Cardiff Locality. Social Prescribing is defined as ‘enabling GPs, Nurses and other health care professionals to refer people to a range of local, non-clinical services’. Building on the experience of previous investment and looking to the learning from other clusters in Cardiff and the Vale, the Clusters within this Locality are looking to introduce a more locally delivered and owned Social Prescribing project targeting the priority health and wellbeing needs of the population. It is intended that this project is formally developed and evaluated with academic input (commissioned separately) from the outset so as to be able to contribute to an evidence base in respect of the outcomes and impact that this model may have on patient reported outcomes and potentially GMS sustainability. The detail of the service model will need to be developed at the point of initiation in conjunction with the commissioned Bidder, but as a minimum the service will need to be able to accept referrals from GP Practices (and other specified services) and include:- - provision of community connectors at cluster level to provide signposting and link patients with community organisations operating locally - the provision of 1-1 and/or group interventions or courses relevant to the needs of the population that is culturally specific with programmes available in peoples first language for the main population groups - integrated working with existing community assets to maximum community capacity to meet the needs of hard to reach groups Should you be interested in participating in this tender, please go to the e-tenderwales portal (link provided in this notice) and access itt_84135

2. Bidders should note that they will be required to enter into terms and conditions of contract as set out in the ITT and that save for matters of clarification or consistency the Contracting Authority will not negotiate the terms.

3. The Contracting Authority will not accept completed ITT's after the stated closing date.

4. All documents to be priced (where applicable) in sterling and all payments made in sterling.

5. All tender costs and liabilities incurred by bidders shall be the sole responsibility of the bidders.
