Abstract

This notice and the corresponding VEAT notice (2021/S 000-024281) provide an explanation and justification for the decision taken by NHS Hampshire, Southampton and Isle of Wight (HSIOW) CCG to ensure continuity of service for the Shakespeare Road Medical Practice. Therefore, it also confirms the intention to direct award a nine month contract with a three month extension option, commencing 1 December 2021, to The Practice Surgeries Ltd (TPS) due to extreme urgency brought about by unforeseeable events by the contracting authority.

The Practice Surgeries Ltd (TPS) has well-established links with General Practice and secondary care providers in the North Hampshire area. As a national organisation, with local presence, they are able to draw on external provision of managerial and clinical workforce along with digital capability to reinforce the primary care services required for this population. The organisation is able to offer and mobilise at scale within the time frame given.

The service specification will remain unchanged from the existing APMS contract requirements. Any significant changes to the current service specification should be preceded by further public consultation undertaking, with consideration given to CCG's estates and workforce strategies.

Background

HSIOW CCG has been working closely with Bramblys Grange Medical Practice to implement a range of remedial measures since October 2019. Reviewing the clinical model, delivery of quality services and financial support. In January 2021 the CCG recruited a remedial team consisting of clinical and managerial/clerical staff working on site and remotely to address the areas for quality and safety improvement enforced by CQC in their inspection of December 2020. This programme of work continues to be monitored via a project team of CCG, Wessex Local Medical Committee, NHS England and practice representatives who have been working closely to stabilise the practice during the pandemic and restoration.

The service is provided from dedicated premises at Shakespeare House with a smaller unit serving 7% of the practice population at Fort Hill in the Winklebury area

Shakespeare Road Medical Practice is a medium sized practice based on the edge of Basingstoke town centre, serving 13,511 patients (raw list size as of July 2021).

Demographic data at July 2020 demonstrates the patient population of Shakespeare Road as broadly equally divided by gender (52% male, 48% female). The Practice is 4% under the National average of 17.4% for elderly (Over 65s) at 13%, with the highest age bracket under 18s who occupy 24% of the registry. National statistics indicate 5% of patients registered with Shakespeare Road Medical Practice are unemployed compared to 2% across North Hampshire.

National statistics indicate that patients in areas of extreme social deprivation can have up to 30% more long-term conditions and appointments than patients registered in areas of affluence, with continuity of care highlighted as a key factor in patient assurance and support.

Value of Fixed Term Contract Arrangement

The overall estimated financial envelope of the interim contract arrangements is circa £2,077,796 per annum (£2,461,076 with the non-recurrent 12 month £383,280 premium applied) based on the weighted list size of 12,776.01 (1st July 2021). Breakdown of the financial envelope is as follows:

Core Contract Value: £1,177,820

Other Income and Pass through Payments: £899,976*

Premium: £30 per weighted patient (12 months only): £383,280

Total: £2,461,076

Other Income (QOF element: £156,811 Full Year): The QOF assumption that has been built in is based on 100% achievement, so is the maximum possible value, although this will be reduced if any indicators are not met in full

Other Income (Enhanced Services element: £104,744 Full Year) is based on prior year outturn, or where the value is calculated on list size the most up to date data has been applied. This will fluctuate based on actual activity, but anticipate that it won't be materially different to the plan number.

Other Income (PCN Enhanced Services element: £279,948 Full Year) is based on prior year outturn, or where the value is calculated on list size the most up to date data has been applied. This will fluctuate based on actual activity, but anticipate that it won't be materially different to the plan number.

Other Income (All other elements: £94,418 Full Year) is based on prior year outturn. This will fluctuate based on actual activity, but anticipate that it won't be materially different to the plan number.

Other Income (Premises: £264,055 Full Year) - these are pass through payments and are based on current the information re: rent valuations and business rates etc. that the CCG are currently reimbursing. This will be subject to change as and when a District Valuation assessment is due.

Should opportunities arise for additional sources of funding during the fixed term contract term (e.g. to pilot new models of care or providing Locally Commissioned Services), these may be pursued by commissioners and/or providers.

Award of a contract without prior publication of a call for competition

Justification for selected award procedure:

Extreme urgency brought about by events unforeseeable for the contracting authority and in accordance with the strict conditions stated in the Directive

Explanation

The patient list was intended to be absorbed into Bramblys Grange Medical Practice on 1st October 2021, however, on 7th September 2021 the Practice formally notified the CCG that they would not be in a position to continue with the plans for the list absorption. Therefore, the CCG confirms the intention to direct award a nine month contract with a three month extension option, commencing 1st December 2021, to The Practice Surgeries Ltd (TPS) due to extreme urgency brought about by unforeseeable events by the contracting authority.

The following points of justification are provided in order to clarify the decision made by NHS HSIOW CCG:

The very short time frame to ensure a provider is sought for the practice

Dispersal of the practice list is not currently an option as it would destabilise neighbouring practices, given the high numbers of patients currently registered at Shakespeare Rd, putting significant pressure on existing staff and estates that could have an impact on the quality of care that patients would receive as a result.

Due to the significant time pressure on the commissioner to identify solutions at a time where general practice is continuing to restore services post Covid, on-going resilience challenges, winter planning and a resettlement programme for Afghan refugees the commissioner, in partnership with its PCN clinical leaders and other system partners, has explored a number of possible options and have taken the decision to direct award the contract.

The CCG will be undertaking an urgent review of options for the continued delivery of services to the patient population of Shakespeare Road Medical Practice. Any decision will comply with the CCGs statutory duties and prevailing Public Contracts/Procurement Regulations.
