The provision of care homes services outside the Borough of Calderdale for Adults with a Learning Disability and / or Physical Disability. There are two types of care home - residential and residential with nursing. The aim of the Service is to provide high quality, safe and personalised care home services that promotes choice, dignity, control and quality of life for all service users. The service must provide good quality care and support services which are efficient and provide value for money to service users.
Placements will be made depending on where the service user choice on where they want to live, as specified in the Care Act 2014.

II.2.4) Description of the procurement
The provision of care homes services outside the Borough of Calderdale for Adults with a Learning Disability and/or Physical Disability. There are two types of care home - residential and residential with nursing. The aim of the Service is to provide high quality, safe and personalised care home services that promotes choice, dignity, control and quality of life for all service users. The service must provide good quality care and support services which are efficient and provide value for money to service users.
Interested Service Providers will complete an Accreditation Application Form for assessing the suitability to deliver services under the contract. Contracts will be issued to Service Providers who pass the Accreditation process and does not carry any guarantee of work as placements will be made depending on where the service user choice on where they want to live, as specified in the Care Act 2014.
II.2.14) Additional information
Applications are to be submitted by the Service Provider for each care home with no guarantee of work. Under the Care Act 2014, service users have full choice and control on where they want to live. The process for requesting an Accreditation Application Form is by requesting through email to ICCQT@calderdale.gov.uk

Contracts are awarded to Service Providers that are registered with the Care Quality Commission (CQC), with an overall rating of ‘Good’ and, is successful in the Accreditation Application Form (AAF) that will be assessed for PASS/FAIL in the conditions set out in the AAF including credit checks and technical references.
