The Supplier shall deliver improved outcomes through the pro-active identification of Carers; Assessment of needs; production of Support Plans; Review of the Support Plan; and provision of support for All Age Carers in Westmorland & Furness.
The Service is part of a whole system approach to the delivery of services, and as such requires agencies to collaborate to provide early intervention, prevent and delay Carers from needing health and social care intervention, aiming to improve the health and wellbeing of Carers and their families.
We and our system partners are committed to:
• Safeguard children and support families and schools so that all children in Westmorland & Furness can grow up in a safe environment, and can fulfil their potential
• Support older, disabled and vulnerable people to live independent and healthy lives
• Enable communities to help shape their local services, promote health and wellbeing and support those in poverty
This specification focuses on:
• Integration of services for Carers through a Whole Family approach that coordinates and improves awareness and access to local support, building and promoting existing networks for Adult Carers and Young Carers across Westmorland & Furness.
• Delivering a collaborative, strong partnership and Whole System approach to ensure improved pathways and outcomes for Carers in Cumberland.
• Supporting the development of innovative solutions to best meet the needs of All Age Carers living in Westmorland & Furness.
• The needs and rights of the Carer in their own right, taking a holistic view during assessment and support planning, with a focus on the impact and support for the Whole Family or others in the Carer’s support network.
• Empowering Carers to achieve improved quality of life and health and wellbeing outcomes and to build resilience to have a strong voice is at the heart of the service’s design, delivery and evaluation.
• Providing a high quality and effective, point of access to Carers offering a range of diverse services to meet their needs.
• Maximising the impact and effect of value the provider can bring.
• Providing Carers with the skills and knowledge to support their Caring role.
The Authority is flexible in its approach to service delivery, always seeking solutions that will offer quality and value for money and working with delivery partners who share its values and commitment to the success of the All Age Carers service.
