II.2.4) Description of the procurement
Procurement for a 3rd party Provider to provide Community Navigator staff (at least 3 staff) to join the Bucks Adult Mental Health Gateway Service to support Adult Mental Health patients to navigate and engage with the voluntary community-based organisations to support their overall holistic wider needs.
The role of Community Navigators within Buckinghamshire Adult Mental Health Gateway is to act as a conduit to Voluntary, Community and Social Enterprise sectors, provide one-off short-term support, goal focused and targeted support to specific Service User cohort who have not met the threshold for Improving Access to Psychological Therapies (IAPT) / Community Mental Health Team (CMHT) specialist service. This is preventative and proactive intervention in meeting the needs of Service Users who would have fallen through the gaps.
It is envisaged that the provision of an integrated Community Navigator service will provide holistic support, connecting people to local initiatives and services for practical and emotional support. The service aims to increase people’s connection with others and their local community and empower them to take control of their own health and wellbeing. The aim of a Gateway Community Navigator provision is to meet the changing and growing needs of the community. To ensure people access the right support at the right time.
