Informed by feedback from patients, the CCGs have concluded that moving towards an accountable care system model is the right model for Bradford. The CCGs intend to work with local healthcare providers and the Council to determine the scope, outcomes and structure of the model through a structured collaboration process. The model will deliver community services, integrated with primary care and local acute services.
The CCGs anticipate a contract with a single entity but this will be explored and tested as part of the structured collaboration process. The preferred model, to be explored through the structured collaboration process, is for the providers (or other interested provider meeting the criteria above) to form a partnership/collaboration or alliance approach; 1 of these providers is to hold (or a single entity to hold) the formal contract for the Services from CCG for the wider provider group. The providers must have contractual arrangements to underpin delivery and governance of the Services and share funding and risks as agreed. The objective is for the providers, through these arrangements to become responsible for managing expenditure of the Services within a fixed budget based on the needs of the population and achievement of healthcare outcomes.
