Abstract

pportunities within the East Riding’s adult population to prevent, delay or reduce the pressure on ASC through early intervention and timely personalised support to address and improve resident’s Mental, Physical and Social health, reduce inequalities and inequities resulting from Access and infra-structure issues in the area. The Council is seeking a collaborative, innovative and altruistic partner organisation, experienced in working with people, communities and partners to deliver time limited, personalised support to residents in a variety of ways and locations.

The new Service is smaller, in terms of the combined staffing requirements of link and connectors, than the previous social prescribing contract. This change in requirement is due to the changes in the ways the Authority’s services are being commissioned and the inclusion of link workers in PCNs and increased capacity delivered by the NHS in its 10 year plan.

This service will be based in the East Riding of Yorkshire and only accessible by adult residents of the East Riding of Yorkshire.

The contract will be for a period of two years initially with options for extensions upto a maximum of two additional years at the Council's discretion.
