This contract will commission an intervention and prevention Young Carers Service for Swansea, ensuring young carers and their families receive timely and appropriate support.
The commissioned provider will deliver a coordinated support offer for young carers, including assessment, safeguarding oversight, emotional and practical support, respite opportunities, youth club and group sessions, one-to-one support, school engagement, awareness raising, and links to wider partnership support. The service will also work in partnership with Social Services, Health and Education to support effective data sharing, joined-up working and access to positive childhood opportunities.
Overall, the service aims to identify young carers at an early stage, provide proportionate support, and help them maintain their wellbeing, education, family resilience and access to childhood opportunities.
