The NHS Wales Shared Services Partnership, hosted by Velindre NHS Trust are acting on behalf of Cardiff and Vale University Local Health Board who sought to establish a contract for Well Being Co-ordinators to improve the interface between GPs and the community to deliver public health priorities and embed public health principles and enhance the social model of care through their use of community networks and experience of co-production. In recognition that approximately 20% of GP attendances are for social/well-being related issues. The service is aimed at adults over the age of 18 years old.
The Well-being Co-ordinators will work closely with the clusters within which they operate and the public health priorities and social prescribing focus and direction will be agreed with the Cluster Community Director, to ensure the Well-being Co-ordinators proactively take forward actions and priorities set out in the cluster plans. The service will ensure effective interfaces are established and sustained with existing cluster based agencies.

NHS Wales Shared Services Partnership on behalf of Cardiff and Vale University Local Health Board will allow a minimum 10 calendar day standstill period between notifying the award decision and awarding the contract.
Unsuccessful tenderers and applicants are entitled to receive reasons for the decision, including the characteristics and relative advantages of the winning bid and the reasons why the tenderer/applicant was unsuccessful. Should additional information be required it should be requested of the addressee in section I.1.
