Description of the contract
This contract is split into 2 lots- East and West
Community activities project – East region
The North Wales Together Programme Team are looking for a Community Provider organisation to co-produce a range of community activities within Wrexham and Flintshire over the next 2 years. The project will enable citizens (including those with more complex needs) to be more connected to and included in their communities, via a range of inclusive community activities, as co-produced with them. The Aim of the project is to enable the full inclusion of citizens with a learning disability into a range of community activities. These activities will be co-produced with citizens and carers and will lead to a range of connections, relationships and roles.
The project will enable citizens (including those with more complex needs) to:
a. Create or strengthen community activities that actively welcome those from the LD community.
b. Develop a stronger personal network of connections and relationships
c. Grow skills, develop ambitions and achieve responsible roles in their community.
d. Maintain or improve their emotional health and wellbeing
e. Be connected and kept up to date, via an accessible and bilingual information sharing platform
f. Citizens and carers should be rewarded for their contribution to the development of the platform wherever that is possible or desirable

The project will enable citizens (including those with more complex needs) to be more connected to and included in their communities, via a range of inclusive community activities, as co-produced with them. The Aim of the project is to enable the full inclusion of citizens (children, young people and adults) with a learning disability into a range of community activities. These activities will be co-produced with citizens and carers and will lead to a range of connections, relationships and roles.
The project will enable citizens (including those with more complex needs) to:
a. Create or strengthen community activities that actively welcome those from the LD community.
b. Develop a stronger personal network of connections and relationships

The contracts will run until 31/03/2025 with the option to extend for 1 year subject to funding.
