We employ 625 staff and we wish to provide health care insurance policy cover for all our staff which would allow them to claim back money from a range of medical services.

General Requirements - Essential

• Option for employee to upgrade at their own cost

• Dependent children included at no additional charge (up to 4 and up to age 24)

• Option to enrol spouse or partner at employee’s cost

• Company Health Report (regular provision of usage stats)

• No Qualifying Periods

• Pre-existing medical conditions to be covered

• 100% Reimbursement Level

• All administration of the scheme to be undertaken by the supplier

Total quantity or scope of tender

Tender contract value

It is envisaged that the tender value will not exceed £100,000 over the two years of the contract.

Benefits and Services to be Included Separately with an annual limit of at least the amount stated below:

• Dental (check-ups/fillings/dentures/crowns/hygienist/x-rays) £60

• Dental Accidents (dental injury treatment as a direct result of accident) £200

• Optical (eye test/glasses/contact lenses/repairs/laser eye surgery) £60

• Physiotherapy, Acupuncture, Osteopathy, Chiropractic (registered practitioner) £150

• Chiropody /podiatry £20

• Medical Consultation £200

• MRI, CT, PET, X-Ray and ECG Scanning Facilities in addition to Consultation at full cost

• EAP - 24hr Counselling & Advice Line to include up to 6 sessions of Face to Face counselling/Cognitive Behavioural Therapy (CBT) as necessary (financial/legal /family advice)

• Access to a second medical opinion or medical advice (access to online GP 24/7 – where GP is able to prescribe a private prescription)

• Personal Accident Protection (accidental death) - £2,500

• Personal Accident Protection (permanent disability) – payment if an injury caused by an accident prevents person from doing any job

• Hospital day case cash back per day - £10 up to 10 nights

• Hospital in-patient overnight stay cash back per night - £10 up to 28 nights

• Birth or adoption payment - £100

• Hospital parental stay (for a parent accompanying a child who has an overnight stay) - £10 up to 28 nights

Failure to meet the above criteria will result in the tender being rejected

3 Conditions for Participation

3.1

Minimum standards and qualification required

Tendering Terms

CCDG reserves the right to reject or disqualify a Tenderer where:

the tender is submitted late, is completed incorrectly, is incomplete or fails to meet the CCDG's submission requirements.

the Tenderer is guilty of serious misrepresentation in relation to its application and / or the process; and / or

the Tenderer contravenes any of the terms and conditions of this tender; and/or

there is a change in identity (including where applicable the make up of consortia), control, financial standing or other factor impacting on the evaluation process affecting the Tenderer; and/or

where the pricing of a tender is abnormally low

The disqualification of a Tenderer will not prejudice any other civil remedies available to CCDG and will not prejudice any criminal liabilities that such conduct by a Tenderer may attract.

CCDG further reserves the right to:

cancel the tender process at any stage for any reason;

require a Tenderer to clarify their submission in writing or by means of a clarification meeting and / or provide additional information. (Failure to respond adequately may result in the disqualification of a Tenderer);

amend the terms and conditions of the tender process; and/or

award only part of the contract.

CCDG reserves the right to withdraw from the tendering process at any stage or to vary the scope of the Products required and CCDG shall not be liable to any Tenderer as a result of this action. In the event that the tender process is cancelled no part of the tender submitted shall be returned to the Tenderer.

Tenderers acknowledge and agree by their submission that their tender remains open for acceptance by CCDG for six months from the closing date for receipt of tenders.

Price Calculation 40%

The lowest price submitted will be awarded the maximum score of 40 for price alone, with tenderers thereafter being allocated a relative score which will be 40 marks less the percentage difference from the lowest price.

For example lowest price is £40,000 the next lowest is £50,000 the second lowest tender would score 40 – (10,000/40,000x100) = -10 score of 30.

• Personal Health Risk Assessment/Health screening (well man/woman screening and screening that helps prevent an illness) - £100

• Home Assistance Cover – Up to 14 hours of paid personal care of domestic assistance after a planned hospital stay of two nights or more

• Health Club/Gym Concession

Criteria 1 scoring 2% per option included within the cover from the above list

Criteria 2 - 15%

• Explanation of how the successful supplier will help to raise awareness of the scheme and its benefits to all members of staff

Criteria 3 – 10%

• Description of claims process and average time for claims to be paid

Criteria 4 – 15%

• Description of how your company meets its obligations under corporate social responsibility and environmental sustainability.

The scoring methodology Criteria 2-4

• Very Good or Fully Compliant Submission -which meets all requirements and is fully explained in comprehensive detail – between 90 to 100% of the marks available

• Good or Fully Compliant Submission -which meets all the requirements and is explained in reasonable detail between 70% and 80% of the marks available

• Satisfactory or Compliant Submission -which meets the essential requirements and is explained in adequate detail 50% to 60% of the marks available

• Weak or Partially Compliant (Minor issues) Submission where some areas falls short of requirements and is poorly explained 30% to 40% of the marks available

• Un acceptable or Non Compliant (Major issues) Submission which fails to meet requirements and is not explained10% to 20% of the marks available
