Title
Individual Review of Commissioned Packages/Placements within the MH and LD Service Group
|

Description of the goods or services required
SBUHB Mental Health and Learning Disabilities Service Group manages the Complex Care of their patient population. This process involves completing comprehensive details assessments of the patients, coordinating Complex Care Panels, agreeing funding allocated to the identified needs and source appropriate placements/packages to meet the needs of the patients presented to panel.
Following on from this approval the service then reviews the packages/placements in line with national guidance to ensure the patients’ needs are being met in line with all the governance requirements put in place.
The service also links with other statutory agencies in relation to funding and commissioning joint placements and completing joint reviews of certain packages/placements.
Aspects of the above are coordinated by the Service Groups Complex Care team in conjunction with the care coordinators within the clinical teams within the various decisions of the Service Group.
Comprehensive data is collated in relation to needs and outcomes of the reviews in line with our governance requirements.
We require assistance to support the Complex Care Team in their reviews of the packages/placements. This would require a service that has a comprehensive understanding of the national CHC Frameworks, awareness of the structures the service group commissioning and working knowledge of the requirements of the reviews that would need to take place and what those reviews should focus on in line with national guidance.
