Abstract
Primary Care is the frontline in terms of how people access healthcare and advice. But with increasing demand on health professionals time, increases in chronic illnesses such as diabetes and heart disease, the focus is less and less on staying well, but instead on treating disease, and acute illness. There are many requests coming from Welsh Government and Public health, asking GP Practices to take part in health promotion initiatives, such as Dementia Awareness, and Cancer Screening, not to mention eating well, regular exercise, and cutting back on your alcohol consumption!
The Mid Powys Cluster would like to develop a role that addresses how we can support people in our communities to have better health outcomes. Not focussing on their illnesses, but on giving them the skills and knowledge to stay well, and also, to know where to get support if they need it.
