Abstract

Hywel Dda University Health board has a tender requirement for provision of a series of patient education / support films, covering initial topics of: • How to access Primary Care Services • Hypertension • Asthma These films would complement our existing library and would need to be easily accessible and embedded into existing clinical systems, so they can be prescribed to patients and recorded for audit / service improvement purposes. The project aims are: Film 1 – Hypertension (3 to 5 minute film) NHS Wales and the British Heart Foundation Cymru report that one in five adults in Wales are being treated for high blood pressure (hypertension). There is estimated to be many more who are undiagnosed and untreated. High blood pressure is one of the leading risk factors for premature death and disability in Wales according to the Global Burden of Disease study. At least half of all heart attacks and strokes are associated with hypertension and it is a major risk factor for chronic kidney disease, heart failure and cognitive decline. Treatment is very effective in lowering blood pressure and improving outcomes. One of our key aims is to work with partners to promote public awareness and opportunities for testing at home or in partnership with Primary Care contractors. This first film would examine hypertension and help people to understand the risks, how a change in diet and activity could make a difference as well as the importance of taking medication regularly. Film 2 – Asthma (3 to 5 minute film) Advice and support for asthma management is important for those learning to live with this condition, which can be frightening and unpredictable for them and their families. In addition to the resource available within primary and secondary care, the expertise within pharmacy settings is considerable. No appointment is needed, opening hours are often more flexible and increasingly pharmacies have a private consultation room. Help is available with inhaler technique as well as prescriptions. A free 20-minute consultation is available (a medicines use review) as well as an Asthma Control Test to help assess the effectiveness of an individual’s self-management. The second film will examine asthma and help people to understand risk, and how to access the support of their GP, and also how their pharmacy can support them. Film’s 1 and 2 are aimed to help people to understand what is happening to them and how they can start to work with their clinicians to play a greater role in self-managing their health. Peer to peer sharing of experiences along with explanations from medical experts should be used to help engage the audience. Language should be straight forward. Film 3 – Clinician’s ‘How to’ film (1 to 2 minute film) The means to prescribe short films to patients, with a short ‘How to’ film, which shows all clinicians across Primary Care how to access these videos and prescribe them to their patients. Films could be split to include: •Self-care / Self-referral – e.g. NHS 111 Wales website •Dentists – patients to be informed how to access services provided, that dentists can prescribe antibiotics and pain killers for dental conditions and these requests should not go to the GP. How to find nearest Dentist. •Opticians – as well as eye tests, many areas run local NHS services with specially trained community opticians for minor eye conditions. How to find nearest Optician / access services. •Community Pharmacy – offers a range of services to promote self-care and reduce demand in general practice. Common Ailments Service. Provide advice for patients starting new medicines for long-term conditions. Film 4 – How to access Primary Care Services (20 minutes’ worth of film content)

A series of films, which educate people on the right way to access primary care services: ‘Right person, right place, right time, right care’. We want people to seek support from the appropriate service whether that should be NHS 111, GP surgeries, Community pharmacy, Dental, Optometry Services etc.

All films must be illustrated in both Welsh and English, with bilingual participants so that the resource works seamlessly in both languages.

Finally, support with an evaluation to understand how many films have been prescribed, clinical satisfaction with the process and the impact on both patients and clinicians.
