Description of the goods or services required
Wrexham Council’s vision for extra care is for a vibrant community that maintains or enhances people’s skills, functioning, self-confidence and interests. People are encouraged and supported to participate in outside activities and interests and maintain outside relationships. Our ethos is one where occupants are as much as possible in charge.
The scheme provides specialist housing for older people in the Borough, promotes independence and supports occupancy sustainment. It helps to reduce social isolation amongst older people and prevents or delays people from having to access high levels of social and health care whilst providing a sense of safety and security for individuals and communities.
Wrexham County Borough Council invites tenders from suitably qualified and experienced Organisations to provide services for vulnerable adults over 55 with a need for either Housing Related Support or a Domiciliary Care Service at Plas Telford, the Council’s Extra Care Scheme in Acrefair, Wrexham, LL14 3AU
The site has in total 54 apartments: 42 with two bedrooms and 12 having one bedroom. Half of the apartments will be occupied by tenants who have no personal support needs. Each tenants’ care package will be based on an individual assessment of need.
Applicants must be registered with the Commission for Social Services in Wales (CSSIW), and be able to provide domiciliary care services and housing related support services that will enable Service Users to sustain residency within the Extra Care Housing Scheme, whilst preserving maximum independence, minimising risks and promoting quality of life. The successful applicant will be expected to provide services within this context to:
• support vulnerable adults to continue to live independently at Plas Telforf Extra Care Scheme through the provision of Housing Related Support.
• support vulnerable adults to continue living within their own homes at Plas Telford Extra Care Scheme, through the provision of Domiciliary Care
• reduce social isolation and enhance the health, well-being and quality of life of vulnerable adults living within the Development.
• treat people as individuals and promote their dignity, independence, empowerment and social inclusion.
The Contract will be let for an initial period of two years, with an option to extend it for 2 further years in yearly increments, and will commence on the 25th September 2016. The successful Tenderer must be able to staff the service from the 1st September in order for managers and care staff to attend training and become familiar with the layout of the scheme prior to commencing any packages of support.
The successful Provider will have access to an on-site office for the purpose of delivering the service detailed in the Contract Terms and Conditions and Service Specification and will be required to have as a minimum one member of staff on-site between 7.00am – 11pm 7 days per week
It is predicted that the purchaser will commission in the region of 9000 hours of support annually on a spot contract basis.
Currently the scheme does not warrant a staff presence overnight but it may be necessary to implement such a service if dependency levels increase over the lifetime of the contract.
Tenderers must submit a fixed price for an hour of service provision. Tenderers must submit a price sustainable for the duration of the initial contract period of 2 years plus the extension period of 2 years maximum)
