Title
Business Case for Electronic Advance & Future Care Planning Application

Description of the goods or services required
The purpose of this procurement is for the NHS Wales Health Collaborative to provide consultancy agencies with a brief on requirements of the End of Life Programme in order to develop an Outline Business Case for an Electronic, Advance and Future Care Planning Application.
The Health Collaborative are looking for a project consultant with experience of NHS Wales to develop a full Outline Business Case (OBC) for a National IT based solution to record individuals’ Advanced Care Plans (ACP).
The OBC will be submitted to the End of Life Care Board, the NHS Wales Health Collaborative Executive Board and the NHS Wales Informatics ‘Digital services for patients and the public Board’ for endorsement The signed off OBC will then be presented to Welsh Government to agree a funding method for development work moving forward.
In support of the End of Life Delivery Plan and the NHS Digital Health and Care Strategy, NHS Wales will require an OBC that includes an IT solution
