Abstract

Hywel Dda University Health Board seeks to commission a suitable provider to deliver a bespoke service to help patients in the North Ceredigion Cluster area to better manage their weight and consequently lower their risk of comorbidities associated with obesity, such as cardiovascular disease, cancer, diabetes, and mental health complexities. Hywel Dda University Health Board seeks to commission a suitable provider to deliver a bespoke service to help patients in the North Ceredigion Cluster area to better manage their weight and consequently lower their risk of comorbidities associated with obesity, such as cardiovascular disease, cancer, diabetes, and mental health complexities.

By partnering with a suitable provider, the Cluster wishes to create a funded pathway which patients can be referred to for physical support, as well as providing them with the necessary information so they can combat their obesity with healthy eating plans and have access to clinical dieticians for ongoing weight management, and diabetic nurses for necessary clinical support.

The Provider will be expected to provide a rolling eight (8) week tailored exercise programme for referred patients. Each cohort should accommodate a maximum of twelve (12) patients per session, with each patient having access to two (2) exercise sessions per week. The programme will be tailored to the respective patients following an initial assessment by the Provider to best determine the patient’s individual needs and goals.
