By partnering with a suitable provider, the Cluster wishes to create a funded pathway which patients can be referred to for physical support, as well as providing them with the necessary information so they can combat their obesity with healthy eating plans and have access to clinical dieticians for ongoing weight management, and diabetic nurses for necessary clinical support.
The Provider will be expected to provide a rolling eight (8) week tailored exercise programme for referred patients. Each cohort should accommodate a maximum of twelve (12) patients per session, with each patient having access to two (2) exercise sessions per week.
