Abstract

Welsh Government’s national plan for health and social care A Healthier Wales was published in June 2019 and sets a clear vision for the delivery of transformed, seamless health and care services across the country. Regional Partnership Boards (RPBs), established under the Social Services and Wellbeing (Wales) Act, have a key role in driving the required change in their areas and in doing so are able to access resources from a national Transformation Fund totalling £100m. In early 2019 the West Wales RPB (covering the footprint of Hywel Dda University Health Board) was awarded £12m to support three significant transformation projects forming part of its Healthier West Wales transformation programme. The initial funding period was until 31 March 2021. Subsequently, Welsh Government has awarded the RPB an additional allocation of £6m to support an extended period of delivery to 31 March 2022. The funded projects are: Programme 1: Connect - Individualised wellbeing assessment and personal stay well plan - Regular, proactive calls to check on individuals wellbeing and direct to appropriate support at an early stage. - Tablets providing access to ‘virtual communities’ - A low level 24/7 response service available to participants across the region to respond to non-medical emergencies Programme 3: Fast-tracked, consistent integration - Fast Access Community Teams in all parts of West Wales providing multi-disciplinary to people in their homes, delivered within 2 hours and over a maximum period of 8 days - Complements the welfare response service within the Connect programme - Supported by integrated project managers located across the region with a broader remit to accelerate integration across all parts of the system. Programme 7:Creating connections for all - Connect to Kindness campaign to celebrate and incentivise acts of kindness within communities and organisations - person to person digital time-banking platforms which support groups and individuals within communities to request and provide support to others - Development of ‘local action hubs’ promoting volunteering and supporting active citizenship within specified local communities - Enhancing and supporting community based roles through accelerated skills and community connector programmes. - Supporting asset based community development and community resilience planning through a number of co-production pilots and grant schemes including; participatory budgeting (21-22 only) and inter-generational buddying schemes (20-21 only) Further information on these programmes can be found in our original bids via the following link: https://www.wwcp.org.uk/invitation-to-tender/ Interim evaluations have been carried out in respect of the above programmes, in keeping with Welsh Government requirements, reporting in January 2020 and October 2020. Description of the goods or services required
