Abstract

The proposed service will provide Lifestyle Services in a Primary Care setting within Swansea Bay University Health Board footprint. The contract will deliver up to 210 patient referrals. The service supports patients to take pro-active steps to improve the way they manage their physical and mental health conditions based on what matters to them. The service is available to all patients aged 18+ who reside within the Llwchwr Cluster. The contract will fully embrace the well-being goals by achieving a safer future through effective prevention and support services, help the patients improve their lifestyle and health outcomes. The service is flexible to meet the needs of the patients by offering a variety of access methods. Direct Award Process 2 has been considered the most appropriate procurement process for the purposes of this requirement as the term of an existing contract is due to expire, and the commissioner has proposed a new contract to replace that existing contract at the end of its term; the proposed contracting arrangements are not a considerable change; and the commissioner is of the view that the existing providers are satisfying the existing contract and will likely be able to satisfy the proposed contract to a sufficient standard.
