Abstract

The Strategic Programme for Primary Care (National Primary Care Board, Nov 2018) notes that “learning from the first cycle of Pacesetters influenced the development of a whole system, 24/7, transformational model for primary and community care. This has enabled a whole system approach to redesign, driven by national quality standards but with flexibility to respond to local community needs. Clusters are seen as pivotal to the delivery of this model. The transformational model for primary and community care, which is a whole system approach to sustainable and accessible local health and wellbeing care, supports the vision set out in ‘A Healthier Wales’ and is now adopted as the Primary Care Model for Wales.” Key deliverables: 1.Confirmation of the Primary Care Model for Wales (PCMW) as the theory of change (ToC) to assure Steering Group that all key model components (articulated as 13 aims) are clearly captured and adequately described in terms of outcomes/ maturity levels anticipated to result from transformation activity in Wales. This will involve cross-referencing to relevant UK & international literature, including the Pacesetter critical appraisal report. 2.Derivation of logic model sets for each of the confirmed ToC components/ aims, ensuring these reflect differing levels of maturity, are coproduced with key stakeholder (including local authority/ social care) input and take account of identified transformation principles & enablers. 3.Derivation of an evaluation plan from the logic models for each of the ToC components/ aims, again ensuring this is coproduced with key stakeholder (including local authority/ social care) input. 4.Derivation of a reporting pro forma reflecting this plan, for completion by Directors of Primary & Community Care (DPCC). The Cabinet Secretary for Health and Social Services has agreed to allocate £100,000–£116,000 in 2019/20 to tender for the development of an evaluation framework to assess the impact of the Primary Care Model for Wales. Bidders should note this is a budget figure and formal evaluation of bids including commercial proposals will be considered as part of the tendering process.

Description of the contract

The Strategic Programme for Primary Care (National Primary Care Board, Nov 2018) notes that “learning from the first cycle of Pacesetters influenced the development of a whole system, 24/7, transformational model for primary and community care. This has enabled a whole system approach to redesign, driven by national quality standards but with flexibility to respond to local community needs. Clusters are seen as pivotal to the delivery of this model. The transformational model for primary and community care, which is a whole system approach to sustainable and accessible local health and wellbeing care, supports the vision set out in ‘A Healthier Wales’ and is now adopted as the Primary Care Model for Wales.”

Key deliverables:

1.Confirmation of the Primary Care Model for Wales (PCMW) as the theory of change (ToC) to assure Steering Group that all key model components (articulated as 13 aims) are clearly captured and adequately described in terms of outcomes/ maturity levels anticipated to result from transformation activity in Wales.
