The Cardiff and Vale University Health Board seek to commission a suitable provider(s) to deliver Specialist Palliative Care in the community to the patient population registered within Cardiff and Vale. The service will operate within the three geographical localities of Cardiff and Vale. A single provider model operating within a Locality (identified in this process via Lots) will be the preferred model of service delivery.
