Abstract

The goal of this contract is to advise both partners of potential benefits and risks of a model/s of care where residential and care and support at home services and their workforce have been combined. The focus is to tease out what needs to be done, and how partners can best support providers and their workforce to deliver innovative models of care that are person centred, preventative and place based and which: - meet both current and future demand flexibly and sustainably - support the wellbeing, retention, and development of the workforce - facilitate the coproduction of services across the whole system - assist with the integration of health and social care - promote better links with the community and in turn strengthen its resourcefulness
