Abstract

The goal of this project is to advise both partners of potential benefits and risks of a model/s of care where residential and care and support at home services and their workforce have been combined. We are seeking detailed case studies of instances (in and outside of Wales) where: - regulated provider(s) deliver both residential and care and support (either in an accommodation based setting or not) services within a specific locality. - regulated provider(s) and / or commissioner(s) have brought together a workforce from a residential setting (with / without nursing) alongside care and support at home workforce for the purpose of management, developing support networks, learning, and training. - provider(s) and /or commissioner(s) have brought the above workforce together and deployed their skills and capacity interchangeably to: ~ deliver care and support and /or health related tasks in both the care home and the community. ~ extend the range of early intervention and preventative support services offered in the community e.g. short breaks for carers, day care services, medicine management support, 24 hour falls response service, night-time or palliative care support. Our focus for this project is to tease out: what needs to be done, and how partners can best support providers and their workforce to deliver innovative models of care that are person centred, preventative and place based and which: - meet both current and future demand flexibly and sustainably - support the wellbeing, retention, and development of the workforce - facilitate the coproduction of services across the whole system - assist with the integration of health and social care - promote better links with the community and in turn strengthen its resourcefulness. *PLEASE READ SPECIFICATION FOR FURTHER DETAIL*

The research will be required in two phases:

Phase 1 - Desk based research

Phase 2 - Deep dive study

We are seeking a report which can be shared with partners that sets out:

- an executive summary of the findings, conclusions, and recommendations, with an accompanying presentation in PowerPoint format.

- a descriptive overview of the evidence of different models

- recommendations on how the partners could support the development of any described model (including any barriers to be addressed)

Additional Information

The total budget available for phase 1 and phase 2 is £24,999 (inclusive of any applicable VAT) of this quote, including any expenses incurred.

The successful applicant will be required to breakdown the cost of delivering both phases.

The contract will run until 31 March 2022, with a break clause following completion of phase 1.
